Only 11% of Americans correctly estimate their ideal daily calorie requirements, according to one survey. The rest of us tend to overestimate, says Bonnie Taub-Dix, RD, a spokesperson for the Academy of Nutrition and Dietetics. Let's say you assume that consuming 2,000 calories per day will allow you to reach your target weight, but it really takes 1,800: Those extra 200 are enough to keep an additional 20 pounds on your frame.

Portion size matters and it’s easy to fall into the habit of overeating and underestimating protein portions. An extra egg for breakfast, an additional ounce of nuts as a snack, or perhaps one more ounce of cheese on your salad–it all adds up. However, movement is still important. Large studies from around the world have associated sedentary behavior with a variety of poor health outcomes, including increased mortality (14,15,16,17,18). And prolonged sitting has also been associated with an increased risk for diabetes, cardiovascular disease, and cancer (19,20). If you physically aren't able to work out, low impact activity like walking, swimming, or light house work can still be of great benefit to your calorie burn and overall health.  
I’m a USAF Viet vet; 66 years of age; diagnosed with hernias (surgery not required now); nerve impingement that I just fully recovered from (it happened a couple of months before the diabetes diagnosis). Now I have to rebuild atrophied leg muscles from when the nerves were not innervating the muscles in my left side – the feeling in my arm came back the next day, but numbess from waist down on left side for a few months – some chiro, mainly a lot of scans (MRI, cardiac ultr-sound, etc.) to insure that it wasn’t a stroke.

Eat more slowly. Take your time to taste your food and enjoy it. This will not only help you be more mindful of what you are putting in your mouth, but will give you the opportunity to get to know your hunger and fullness cues a little better. Research implies those that take longer to eat - 30 minutes vs. 5 minutes - can reduce feelings of hunger and increase feelings of fullness, regardless of calorie intake and hormonal responses to food (41).  
I am having reconstructive surgery on my ankle in two days (5/16/13) and since I will be on my posterior for the few weeks after – at least no exercising – I’m looking for advise on dropping 20-30 lbs over the never 40 days so rehab will go better…stats – 5’9 204 – formerly athletic build now couch build – 47 yrs old – I have been a “Ferrisee” (better than Ferrasite) since listening to 4HWW and then 4HB – but need refocused guidance – from one and all – If this is the wrong string please let me know that as well.

I have a fight next Saturday June 15th. Im currently 178 this morning, I need to weigh 170 Saturday at 5pm and expect to fight around 8pm. I know I shouldnt cut a lot of water but dont want to train extremely hard this week as im trying to recover as it is. I plan to do my regular training Monday and Tuesday and take off the rest of the week to rest. I started at 192 pounds and have lost them in the last few weeks naturally but have seemed to hit a plateau. What protocol would you suggest I use doc.

You likely already know that if you want to lose weight, cutting out processed foods and sweets is the first and most obvious step. But those healthy-sounding options can be just as bad, too.

Muscle is more dense than fat, which can make you look leaner overall. Once you reach your desired weight, you may not feel as fit and toned as you would if you lost more fat and maintained your lean mass throughout the process. In order to decrease your body fat and get toned, you would have to gain muscle mass, essentially gain weight back, and try to lean out by burning fat. It is pretty counter-intuitive to lose weight from losing muscle weight just to try and gain back more muscle weight in the end.

Probably. If you have overweight or obesity, your risk for many health problems is higher than that of women who are at a normal weight. Even a small amount of weight loss can lower your risk for or even prevent health problems like diabetes and heart disease. Calculate your body mass index (BMI) to see whether you’re at a healthy weight. If you have overweight or obesity, start by making small changes to your eating habits and getting more physical activity.

Keep researching. I’ve recovered from sciatica pain, lost 19 lbs, dramtically reduced my cholesterol and lowered by blood pressure from 137/93 to 125/89 in 3 months ! Read Dr Greger’s book: “How Not to Die” and Dr Campbell’s book: “The China Study.” Watch the documentary “Forks Over Knives.” Look up Dr Ornish, Dr Barnard and Dr Esselstyn. I’m convinced that eliminating all (or almost all) animal protein from your diet is the way to go. There’s a mountain of evidence that shows a whole food, plant based diet can prevent many cancers including prostate and breast cancer and even reverse many major diseases like heart disease (Dr Esselstyn). Dairy is very bad for you, your patients and your kids. But, don’t take my word for it, you’re a doctor, do the research. Dr Greger’s book references thousands of studies. May be the best place to start.

Fiber expands in your stomach and also takes time to digest, both of which help keep you feeling full for longer. Good sources include whole grains, veggies, and whole fruit (not juiced). Healthy fats like olive and nut oils—in moderation—improve flavor, give you energy, and help your body use certain nutrients. Alexandra Shipper added healthy fats, such as avocado, to protein sources like eggs and fish on her way to dropping 55 pounds.

Furthermore, when we studied 9 lean healthy young men and fed them first a ‘balanced diet’ for a week locked in a metabolic ward and then the exact same calories as a ketogenic diet for 4 weeks, they did not lose any body fat (only about 2 pounds of water) despite being consistently and strongly in nutritional ketosis. (Phinney 1983)  And finally, the 10 low carb runners from our FASTER Study had been following a low carb or ketogenic lifestyle for at least 6 months (average blood ketones of 0.6 mmol/L); but while thin, none of them were continuing to lose significant weight (Volek 2016).
So how do you address any—or all—of these problems? The first thing, says Dr. Daniel Maman of 740 Park Plastic Surgery is to address the quality of the skin. Dr. Maman is a Harvard trained, board-certified plastic surgeon who helps men and women address thigh fat issues with both surgical and non-surgical treatments. He says that the thighs are difficult to treat because the quality of the skin in that area is unique.
This means that the more fat you have, the more estrogen you’ll have, and it also seems to work the other way around. For example, estrogen overload can exacerbate hypothyroid issues, slowing metabolism and causing weight gain. If that “estrogen overload” comes from obesity in the first place, this can very quickly become a vicious cycle (you’re overweight because you have thyroid problems, and you have thyroid problems because you’re overweight).
When I was younger, I’d managed to maintain my weight of 130 pounds by being active. I belonged to the dance team and played intramural sports, so it wasn’t difficult for me to motivate myself to exercise. After college, I started a desk job, and with that came long hours, pizza lunches, and plenty of happy hours. I continued to exercise almost every day, but it wasn’t enough and my weight climbed to 153 pounds, which was too much for my 5-foot-4-inch frame.
“The way that I lost my baby weight is doing P90x, Insanity and PiYo at the YMCA—they all have a great system of cardio and plyometrics. The reps are low and the intensity level is high, so when I finish one rep I feel so accomplished to do the rest. I would keep telling myself to do one more. It also helps to have a great instructor; my instructor would literally do the work with you. That’s what makes me want to do more. I am now at a level that I can keep up with our group instructor, which is amazing. I haven’t felt this alive in a long time. I’m a vegetarian, and we love our carbs, so that was a bit hard, especially after doing cardio. You feel like you’re starving. I cut the carbs—no bread and pasta—and just added more vegetables and cold pressed juices to get extra vitamins in my body. I snack when I’m hungry on cashews and peanuts, and hydrate with water as much as I can.”
Although fad diets and crash diets like the 3 Day Military Diet can be tempting, these diets are not your solution for how to lose thigh fat. In fact, these are exactly the type of diets that you want to avoid. Crash diets tend to be highly unsustainable and put your body into starvation mode. When you are in starvation mode, your body actually holds on to the fat because it perceives the threat of low calorie intake. Remember when we talked about the cavepeople? It is important to find the balance between losing thigh fat and not putting yourself into starvation.
